De equipe met een gynaecologische missie is terug uit Djuma. Het is hier bitter koud in vergelijking met onze temperaturen in Kongo, tot 35°C.

Hallo,
Hier zijn we nog een keertje om te bloggen. We hebben een druk maar prachtig en verrassend weekend achter de rug.
Zaterdagmorgen gaven we les. Joost sprak over “prevoir pour prevenier”. Katelijne vertelde iets rond algemene hygiëne, hygiëne in de operatiezaal, preoperatieve zorgen en de nazorg bij vesico-vaginale fistels. Geert gaf les over de anatomie en de oorzaken van de vaginale fistel. Nadien hadden we een tussentijdse evaluatie die vlot verliep.
Mboté, ebwe? Goeie dag, hoe gaat het?
Kitiko bien. Het gaat goed. Althans met onze equipe hier in Djuma.
We zijn de eerste week rustig gestart met 2 operaties op maandag . Twee
geplande patiënten voor een blaasfistel operatie kwamen niet opdagen. De
dames waren huiswaarts gekeerd om de nodige voorbereidingen te treffen
thuis. Een auto zie je hier heel zelden, maar enkele mensen hebben het geluk zich te kunnen verplaatsen per fiets of moto.
Pakken, Herpakken, Wegen, Goedgekeurd

Onze reis naar Djuma is vlot verlopen, althans wat de vluchten en de verplaatsingen zelf betreffen. De binnenvlucht met de MAF was spannend en avontuurlijk, iedereen kon aan een raampje zitten. De korte vaart over de Kwilu rivier met de bac zorgde voor een adembenemend uitzicht.

Djuma komt aardig dichterbij. Op 7 oktober vertrekken we voor een gynaecologische missie samen met Geert Wauters als gynaecoloog, Joost Swen als anesthesist, Marcel Steenmans als OK verpleegkundige en Katelijne Declerck als algemene verpleegkundige.
We kregen een goede briefing, vooral van Ingrid Cluytens waarvoor van harte dank. Het ziekenhuis kreeg al heel wat opleidingen en we zullen ons best doen om de equipe van het “Hôpital Général de Référence Djuma” verder te helpen.
De valiezen worden gevuld, gewogen, te zwaar bevonden, opnieuw geladen….zoals altijd een hele klus.
Met dank aan het thuisfront dat ons telkens weer de mogelijkheid geeft om ons steentje te kunnen bijdragen.
Een lange wachtrij van vrouwen.
Via système Françoise kregen wij de eerste info over deze zending:
"100den vrouwen wachten en internet werkt momenteel niet, dus we kunnen moeilijk bloggen. Verder alles kits!"
Via een aangepast système Françoise kregen we per mail ook dit berichtje uit de brousse:
"hey allemaal,
alles ok hier, wel warm en vochtig...
Helse dag om hier te geraken: op om 5u, vliegtuig op 8u en na een stop op de procuur in kikwit 5 uren onafgebroken jeep waarvan 1u asfalt en de rest piste. Toegekomen om 16u30, alledrie kapot.

Description
After a drop in activities in recent years, it appears that activities in the hospital have once again been on the rise since 2005. We attribute this rise to the fact that the team of doctors has been reinforced by the addition of some dynamic doctors. In keeping with the situation in the country, there had been frequent successive changes within the team of doctors, but the resident paramedical staff and, especially, the local population insisted strongly that the partnership should be continued.The different services are all present in the hospital and it is noticeable that the doctors and nursing staff are all highly motivated. Everything is clean, and there is no overcrowding, yet there were more patients than we had expected after reading the report of the surgeon who led a mission in Kikwit in May 2004.
28 health zones (‘Aires de Santé’) covering 4000km² are grouped together under Djuma Hospital (Hôpital de Djuma), which serves a total population of 155,521. The doctors and nurses have 1 jeep and 3 motorcycles to visit these health zones. According to their statistics, there was a 45% increase in activity in general and 82% more childbirths in these zones in 2004. The number of days spent by patients in hospital is high because patients often come in too late with complications. The reason for this is that they are not referred quickly enough. There is a nursing school associated to the hospital.
Results of the missions.
It is absolutely essential to continue working to improve basic surgical skills and the treatment of patients in the surgery department: there is often a lack of basic knowledge and skills of patient care, mainly because the team consists of very young and inexperienced doctors. The returning surgical teams consider it absolutely essential to continue to concentrate mainly on basic surgery. Other disciplines can be given attention at a later stage, whereby gynaecology can be considered as an exclusive speciality. Parallel assistance and training in anaesthetics are absolutely essential. The hospital has had to face water problems throughout its existence: in partnership with other organisations and authorities (e.g. Energy Assistance and Vivaqua), MWV is helping to solve this basic problem by sending teams of engineers.The partnership with this hospital is running smoothly: Although the team consists of young and inexperienced doctors, they are very interested in working together and wish to do so. Likewise, amongst the paramedical staff there is a high degree of willingness to work together for a general improvement in the present operation and condition of the hospital. Taking account of the hospital’s absorption capacity, we wish to continue with the same number of missions in 2009. It may be possible to add a surgical mission for 2010 and the water supply project must also be completed by then.
