aangekomen in Kigali
Gisteren een vlotte vlucht naar Kigali gehad. Dr Emmanuel, directeur van het ziekenhuis, is op de luchthaven om ons te verwelkomen. Het is een aangenaam weerzien en de missie gaat reeds van start: we keuvelen wat over gezamelijke kennissen, het rwandese weer maar ook reeds over fracturen en technische orthopedische problemen.
Straks zullen we onze reis naar Mibilizi aanvatten. We moeten dus naar een uithoek van Rwanda: een lange autoreis met continu mooie zichten die nooit vervelen, door het Nyungwe-woud, langs de theeplantages om dan af te dalen naar het Kivumeer en uiteindelijk terug de bergen in te rijden. Daar zullen we ons verblijf voor de volgende 2 weken bereiken op het Rwandese platteland, alhoewel dat laatste eigenlijk een contradictio in terminis is.

2° week in Mibilizi
We zijn in onze tweede week van onze orthopedie-missie in Mibilizi. De laatste dagen is het weer hier prachtig en ook het internet is 'stable'. We hebben al veel gewerkt en merken een evolutie ten opzichte van de vorige, eerste orthopedie missie.
Over de werking van het radiologietoestel was voor de missie wat bezorgdheid, maar het toestel functioneert naar behoren.
In ons huis worden we goed verzorgd door Batiste die elke dag heerlijke maaltijden voor ons tevoorschijn tovert. Dit alles in de prachtige Rwandese omgeving.

Aangekomen in Kigali
Gisteren zijn we vertrokken voor onze missie orthopedie in Mibilizi. We hadden een vlotte vlucht naar Kigali.
De ochtend in Kigali is aangenaam fris; de hemel is wat bewolkt maar het is droog. Straks vertrekken we voor een lange rit naar Cyangugu, weg uit de grote moderne stad met gemakkelijke internetverbindingen. Als het mogelijk is, zullen we nog een blogberichtje sturen met onze ervaringen in het ziekenhuis.
We zijn op het einde van het regenseizoen: hopelijk zijn de wegen ‘un peu praticable’ zoals men hier zegt.

Description
The hospital from the district of Cyangugu is a public hospital and one of the four hospitals which the district of the former province of Cyangugu numbers.The hospital has 155 beds and 3 generalists and is dependent upon the bishopric. A number of sisters still work here, but there is no particular order of nuns which is responsible for the hospital. As regards its finances, the hospital receives a small amount of support from the state (which pays the salaries of 20 of the 72 staff). The number of inhabitants whom they cover is presently 226,732.
The hospital infrastructure has seen better days and does need a coat of paint in many places. The outside looks quite good, but inside there are flaking walls and sometimes whole pieces of the walls have been displaced. The staff are assisted by sisters who ensure that everything runs smoothly (they are often heads of departments). There are no specialists or physiotherapists. There are no sheets on the beds (there are blankets, as it can get very cold) and several mattresses are torn or worn out.
The maternity unit is the busiest department. It numbers 65 beds and the majority of the patients in the hospital are women and newborn babies. The hospital would like to open a neonatology department: they have already received two incubators, but would like to improve the department with some basic material and staff to give newborn babies more care. They would therefore be interested in a gynaecologist and a midwife/neonatologist. The gynaecologist would carry out gynaecological operations and the midwife would be responsible for obstetrics and neonatology.
Results of the missions
The maternal mortality rate is high (5/1000), as is the percentage of Caesarean sections (52%). The vacuum extraction figure is too low: there is a clear need for basic obstetric training. There is a lot of work to be done in this centre and the staff want to see changes, but there is little discipline within the hospital. There is not really anyone who is in charge of the different departments. The hospital has too many staffing problems (the doctors are always new and young) and financial problems for it to develop quickly. Some progress can be noted, but it requires a lot of time and therefore we are planning only two missions a year until 2010.
