Amper enkele weken terug thuis, lees ik met verslagenheid de mail die ik vanavond ontvang uit Goma: dokter psychiater Christine is vandaag overleden. Zij was Congolees arts en specialiseerde zich in de psychiatrie, in Zwitserland. Zij was een heel knappe psychiater en was zonder twijfel de steunpilaar en trots van het ziekenhuis.

Eindelijk is het dan weer zover! Zaterdag vertrekken we opnieuw richting Afrika!
Zoals steeds de week voor ik vertrek, is er opnieuw toch een beetje stress... Ik heb vorige week de thema's doorgekregen waar de verpleegkundigen uit Goma les willen over krijgen en ik ben sinds dit weekend serieus in aktie gekomen wat de voorbereidingen betreft. Gelukkig mag ik steeds op mijn papa rekenen om mijn slides op taalfouten te verbeteren. Mondeling Franse les geven, dat lukt nog wel. Met de nodige omschrijvingen en gebaren kan ik me goed genoeg uitdrukken om iedereen duidelijk te maken wat ik bedoel. Maar geschreven taal die geprojecteerd wordt voor een hele groep, dat moet toch in orde zijn hé! Bij deze dank je wel papa :-)

Mon métier du coeur
Jeune de 13 a,s j'ai déjà voulu être infirmière. Je jouais déjà que je travaillais dans un petit dispensaire. Comme aînnée de dix enfants, je prenais soin de mes petits frères et soeurs. Finalement en 1986 j'ai pu suivre une formation vers mon métier de coeur: infirmière psychiatrique: cela me permet de soigner la globalité de la personne. Et cela me permet de travailler pour Caraes. Dans 'Approches' (Magazine du personnel des Frères de la Charité) il y a un article qui décrit mon engagement, n'hésitez pas de le lire.

Je suis revenu dans mon pays d'origine pour y aider mes compatriotes qui ont
besoin de soins psychiatriques. Je vais commencer à établir le dossier
des patients, en collaboration avec mes collègues locaux et expliquer à
ceux-ci comment en appliquant un traitement pluridisciplinaire, on peut
accélérer la guérison des patients.

Description
In the extension to our partnership with the ‘Shirika la Umoja’ centre for the handicapped, the Brothers of Charity asked us to visit the ‘Tulizo Leto’ centre for mental health care. The name of the centre can be translated freely as ‘quiet place’.
The centre is about 10km from the centre for the handicapped: it is mainly adults who are taken care of here.
Originally, the centre was also part of the ‘Shirika la Umoja’ centre for the handicapped, but in 1989 a division was implemented between care for the physically and mental health care.
In June 2005, this centre was also taken over by the Brothers of Charity at the request of the bishop of Goma.
This centre is the only psychiatric centre in North Kivu. It offers hospitality, outpatient care, and consultations. It is geared mainly to adults, but children can also be seen in exceptional cases (especially those suffering from epilepsy).
In 2006, there were about 1,900 cases for consultation, about 1,000 of which were new.
About 412 sick people were hospitalised for an average stay of 27 days. They obtain their medication from ASRAMES (an NGO which supplies the region with medication and consumable goods at acceptable prices). Dr. Paolo thinks this is the best way of working because they have medication at their disposal (sometimes there is a rupture in the stock) as does the WHO which provides psychiatric support. He is also a strong advocate of following the WHO medication protocol because this offers the best guarantees for psychiatric patients in Africa, for example, in order to be able to guarantee continuity in their treatment. Medication can be requested from Europe in some exceptional cases.
There is a good patient file (see the appendix, for example) which Dr. Paolo also uses to obtain statistical data.
A total of 28 people work here (medical and paramedical staff). One of them is Dr. Paolo Volta, an Italian psychiatrist who has worked here for 3.5 years: his contract (with an Italian NGO) is due to expire at the end of 2007 and, amongst others, Dr. François Pole will come here to continue the work: he has already done work placements in Ndera and Bukavu and is now going to Kinshasa for another 6 months for further training courses.
In 2008, the centre will be incorporated with CARAES into the new six-year programme of the DGDC.
Results of the missions
Training for the local partner is the most important priority. This training must be directed at both doctors and nurses. There are only a few psychiatrists in Congo and they usually stay in Kinshasa. There is a need for a neurologist who can teach them to use the EEG apparatus which they have at their disposal, but also for lessons in the treatment of epilepsy and Parkinson’s disease, for example.
A second need for training is mainly in the area of ergotherapy: occupational therapy which is clearly aimed at social reintegration.
A mission for neurology and ergotherapy is planned for 2009 and 2010 at the request of the local partner.
